Preamp troubles w/ Apollo Twin-X-Duo...

I've had the Apollo Twin-x-duo for about 6 months. About 4 days ago I walk into my studio room and see channel 1 w/ about 3 bars signal. I thought, "Oh maybe I left a channel on in my DAW or something. I examine and find I have no active channels armed or anything that should be creating a signal on Channel 1. I load up a Luna session and arm a track for channel 1 record and I get a disturbing electrical noise. There isn't even a mic signal coming through. The unit is extraordinarily hot. I'm used to the warmth of the Apollo Twin and understand it uses its chassis as a heat sink but this was really odd temperature.
I change mics, cables, reboot everything and go through the "What's causing this?" examination. For the first 2 days it was just channel 1, then channel 2 began the same issue with out plugging into the interface started showing 3 bar signal like channel 1.
I end up calling Sweetwater as it is all under seller/manufactures warranty. They were great at troubleshooting etc. and ultimately had to send the unit in for repair. I've loved everything about UAD/LUNA/and my apollo.
I'm really wondering what I did or what happened. I've read similar instances where a person had repeat issues Apollo Twin X Damaged twice in a row | UAD, Apollo, and LUNA Forums (uadforum.com)
The issue they had was very similar to mine and I don't want to continue a bad pattern.
For the record - I never "hot-swap" mics, never leave phantom power on unless in use and always turn off when done to prevent hot mics and feedback etc... , I only use one condenser mic (AT-2050) plugged into channel 1 as it is my main mic by the desk set up and ready to go.
Line order goes -- Mic AT2050 -> XLR cable -> Apollo Twin.
If anybody can expand on this issue with suggestions or what might've catalyzed the issue it would be greatly appreciated.
